MANSFIELD – The following restaurant inspections with critical violations were conducted by Richland Public Health between June 25 and June 30.

• China Buffet, 2216 Richland Mall Drive, Ontario, June 25. During Inspection PIC did not ensure employees are properly maintaining temperatures of TCS foods during cold storage (critical, corrected). Observed multiple products on buffet cold holding area to be out of temperature. PIC corrected all temperature issues. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected). Facility did not have an employee health policy available for review. Provided RPH’s employee reporting illness agreement to PIC for employees to sign. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed multiple products (sugar, soy sauce, seasonings, etc.) on the cook line to be uncovered and not protected from contamination while not in use. PIC properly covered all products. Equipment food contact surfaces or utensils are unclean (critical, corrected during inspection). Observed the can opener blade to contain food residue. PIC properly cleaned and sanitized can opener. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed TCS foods in the cold holding buffet unit to be 62-69 degrees Fahrenheit (F). PIC voluntarily discarded products: cut lettuce, hard boiled eggs, shredded cheese, ranch, mayonnaise dressing. Tapioca and chocolate pudding had just been opened from room temperature package; these products were placed in walk-in cooler to bring to 41 degrees F before placing on buffet. Equipment and/or utensils improper construction (critical, corrected). Observed salt stored in a re-used container that was not smooth and easily cleanable on the inside surface. PIC moved salt into food grade container. No approved backflow prevention device on the plumbing system (critical). Observed a hose in the dish area and mop sink with a spray nozzle attachment with no proper backflow device. Discussed discontinuing using spray nozzle attachment or obtaining a proper backflow. Correct by July 9. Plumbing system components not constructed or repaired with approved materials (critical). Observed a non-food grade hose being used in the dish area to rinse dishes. Discussed discontinuing use of hose or obtaining a food-grade hose. Correct by July 9.

Weiner King/Sunset Fast Foods Inc., 118 Lexington Ave., Mansfield, June 25. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Employee illness agreement provided by sanitarian at time of inspection. Food not properly protected from contamination by separation, packaging, and segregation. Container of shredded chicken observed in walk in cooler without cover and exposed to splash, dust or sneeze (critical, corrected during inspection). PIC voluntarily discarded less than one pound of chicken. Equipment food-contact surfaces or utensils are unclean. Can opener blade had visible residue (critical, corrected during inspection). PIC had cleaned. Working containers of poisonous or toxic materials not properly labeled. Two spray bottles with cleaner and without label observed (critical repeat, corrected during inspection). Presence of live insects. Flying insects observed in kitchen (critical). Correct by July 10.

 Panera Bread No. 4787, 1402 Lexington Ave., Mansfield, June 25. Refrigerated, ready-to-eat, TCS foods not properly date marked. Sliced tomatoes in make table were not date marked at time of inspection (critical, corrected during inspection). PIC voluntarily discarded two pounds of tomatoes. Working containers of poisonous or toxic materials not properly labeled. Spray bottle of cleaner was not labeled (critical, corrected during inspection). PIC removed bottle to label. 

• Speedway No. 3624, 2132 W. Fourth Street, Mansfield, June 26. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). At time of inspection, Buchick Roller in walk-in cooler was observed date-marked June 24, 2020. Listed item voluntarily discarded by PIC 

• Burger King Restaurant No. 585, 851 Ohio 97 W., Bellville, June 26. Equipment food-contact surfaces or utensils are unclean. Order screen had excessive build-up (critical, corrected during inspection).

• Ashland Road Duchess No. 1103, 1000 Ashland Road, Mansfield, June 26. TCS foods not being cold held at the proper temperature. Observed open display case reading 48 degrees F (critical). Unit shall maintain 41 degrees F or colder at all times. Ordered corrected by June 29. 

• Love’s Travel Stop No. 711, 976 Ohio  97, Bellville, June 29. Handwashing sink is being used for purposes other than handwashing; observed a Monster energy drink in the hand sink in the processing room (critical, corrected during inspection). Food display not properly protected from contamination by consumers (critical, corrected during inspection). Food display not properly protected from contamination by consumers.

• Firenza Pizza, 2074 Walker Lake Road, Ontario, June 29. Food employee(s) did not wash hands when required (critical, repeat, corrected during inspection). Observed an employee work the register, touch face and then go to work with food without washing hands first. Discussed proper handwashing frequency with employees. Employee washed hands. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed the products on the prep line to not be covered when not in use. PIC and employee state the lids were being washed and sanitized. PIC placed lids on all products on prep line. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed a sanitizer bucket between 0-150 ppm. Employee replaced sanitizer solution to bring to 200 ppm. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed a pan of cooked chicken wings in the single door prep cooler to be date-marked June 20. PIC voluntarily discarded chicken wings. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed a container of pizza sauce labeled prepped on June 6. PIC voluntarily discarded product. 

• Lexington-Shelby Elks Lodge No. 2554, 320 E. Main St., Lexington, June 29. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Container of sliced limes in cooler observed uncovered at time of inspection. 

• Mansfield/Ontario Fairfield Inn and Suites,1065 N. Lexington Springmill Road, Mansfield, June 29. Food equipment surfaces not cleaned at required frequency (critical, corrected during inspection). Observed a used coffee filter with coffee beans in the coffee maker. Coffee and filter appeared to contain mold. PIC discarded coffee filter and placed coffee machine part in three-compartment sink to be properly washed, rinsed and sanitized. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed a container of what appeared to be gravy and a container of batter to not be date-marked and was observed to be spoiled. PIC voluntarily discarded products. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed a bag of hard-boiled eggs with a use by date of April 17, 2020. PIC voluntarily discarded product. 

• Deer Ridge Golf Acquistion, LLC, 900 Comfort Drive, Bellville, June 29. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Milk in the bar area was not date marked.
